Overview
As an admin user, you can view a survey participant's results for a non-confidential survey and add additional notes for each survey question or the entire survey in general.
Survey notes are especially helpful in scenarios involving exit surveys and exit interviews. The Survey Notes feature can be used actively during an interview to capture important information.
Alternatively, interviewers can use the Survey Notes feature before conducting interviews to better organize and focus their questions.

Requirements

Grant Permissions
By default, full admin users have access to survey notes for non-confidential surveys.
Non-admin users can be granted survey notes permissions in the survey's Access Settings.
To grant a non-admin user Survey Notes permissions:
- Click and expand Analytics in the left-hand navigation menu
- Click Surveys & Pulses
- Click on the non-confidential survey to access the survey's analytics
- Click Access Settings in the top-right
- Click and navigate to the Access Roles tab
- Enable the following options: Reporting Access: Assign Access > View Invited Participants > View Individual Surveys > Access Notes on Individual Surveys
- Under Manage Users in this Role, click Select Users
- In the Add Users field, enter the user(s) who should be included in this role
- Click Save
Access Survey Notes
To access Survey Notes for a non-confidential survey:
- Click and expand Analytics in the left-hand navigation menu
- Click Surveys & Pulses
- Click on the non-confidential survey to access the survey's analytics
- Click and expand Reports in the header row
- Click Response Rate
- Scroll past the Surveys Launched Over Time card to reach the Invited Participants card
- Click a survey participant's corresponding View or View with Notes button to see the user's survey responses
- The View with Notes option appears for participants with existing notes
- Click Add Notes to expand a text field for each question and an overall field for the entire survey
Create & Manage Survey Notes
With survey notes, you can capture additional notes and information for each survey participant's results. These notes can be general or be tied to each of the survey's questions.
Notes created for a survey question or in general are attributed to the author along with the date of the action. As you enter information and context for a survey, your notes are auto-saved as they're entered.
To create a Survey Note:
- From the non-confidential survey's Response Rate report, click View for the survey participant
- Click +Add Notes in the top-right to expand a text field for each question
- Enter notes or additional information in any of the text fields
- Click Add Note
To manage a Survey Note:
- From the non-confidential survey's Response Rate report, click View with Notes for the survey participant
- Click the edit icon for an existing survey note
- From the editable text field:
- To edit the note: Make the necessary edits in the text field and click Save
- To delete the note: Click Delete Note

Survey Notes via Results Export
Survey Notes can be accessed in the Results export via a dedicated Survey Notes tab. This tab includes the note(s) and the notes' author(s) alongside the original question for the necessary context.
Survey Notes PDF Export
A survey participant's responses can be exported as a PDF file, both with and without additional survey notes.
In addition to the survey participant's name, survey, and date of completion, the exported survey response PDF includes the survey participants' demographics specified to appear on the user's Profile Card, learn more about the Profile Card.
To export the survey responses as a PDF:
- From the Response Rate report, click and expand the PDF Export drop-down in the top-right
- Click either Responses only or Responses and notes
- Click and open the downloaded PDF from your browser
