Paper Surveys

Paper surveys are available for the Best Places to Work surveys. During the survey set-up, you will be asked if all of your employees have email addresses. If your answer is "no" you will be able to indication how many have email addresses, how many will need printed instructions, and how many will need paper surveys. 

If you indicate that employees will need paper surveys, you will have a "Print Paper Surveys" in your online portal. This tab will include the mailing address for the completed paper surveys and the deadline for submitting them. Keep in mind there is a $2.00 charge per survey to cover the cost of data entry. 

You can download and print the surveys in either English or Spanish. Simply click on the download button and print as may copies as needed.
